The Membership Information Form is for submitting new members, deleting members, or changing member information with Kiwanis International. In this version, fields are fillable and printable once filled in, but cannot be saved with data.
Looking for one universal form, are we going back to this old format?
The current form is now fillable, it can be saved to your computer with current version of Acrobat Reader 8.0 or higher (available as a free download on the Adobe Web site if needed), and e-mailed, faxed or postal mailed to the International office.
This form can be used for many things. It can be used to add, delete or transfer mambers. You can also update member information with this same form.
It was easy to find this from the "forms and downloads" link. However I don't understand why there are two different forms, "Membership Information" and "New Member Application", which both seem to do the same thing. Personally I prefer the form at this link.
In these days and times of electronics, we should not have to snail-mail any forms! I have numerous member e-mails, etc, to update for my club, and it sure would be nice to sit at my computer and update right off of the member list. Do I have the option of e-mailing any changes?

Ken - Thank you for the feedback. We will be redesigning this form for online usage and that form will have a mobile phone option.
Please remember that in order for a New Member to be added, clubs really are supposed to send the New Member Add Fee along with the form. This fee is $50 for Kiwanis Internationtal + any new member add fee charged by your district. When you e-mail in a New Member form, you should be printing the form and mailing it along with a check the same day. Please make sure to do this to help out the KI staff in tracking fee payments, figuring out what the hundreds of checks they receive each day are for, etc. Also, if the check clears - you know KI got the New Member Add form. It's amazing how much mail is lost by the USPS! Thanks!

If a member is changing from membership to friend, how do I show that?
Carin, Kiwanis does not have a membership designation of "Friend". The Kiwanis International board is willing to review recommendations for membership designations, please submit your definition of the classification of "Friend".

Question. Where does it distinguish on the form for corporate membership applications? I don't see that referenced anywhere
This is a good form. However, please ask that the Kiwanis membership Information Form (application), which is included with "Changing Tomorrows Today" asks all the questions the secretary is required to answer on this downloadable form.
One example: It makes a difference if an applicant is a former member of a Service Leadership group. I just learned that the KI new member fee is waived in such case, also that dues for 2 years are discounted. Our newest member is a former SLP group member, but we sent in the full $50 KI fee, without knowing the benefit. I hope she can get a refund. This question about former membership is NOT on the form an applicant submits to a club.

My District has indicated that this is now available.
George - There is no separate application for a corporate membership. On the membership application, the club only needs to note the membership is a Corporate Membership. If that member changes, KI only asks that the club report the change in information.

we have the old form and still thesame. We've taken from the manual kit 2 years ago and everytime we need we only have to re copy.
To add a new member on the Kiwanis Membership Information Form, after downloading, filling it in and saving, who should the emailed form go to? Still need to send in a hard copy with the check.....

What does a corporation get out of a corporate membership? In other words, what's in it for them? How does the club recognize/publicize the company as being a corporate member. I understand their name is only referenced-to on the membership application form. There needs to be more done to show our appreciation to them for becoming a member and to let the community know they are a member. Is this strictly up to the club?
I see mention of "corporate membership" . Where do I find more information on this form of membership?

Before it would not save cause the PDF profile had this excluded. I had to make bit maps of the completed form and save the picture.
