This is a good template, but it will not allow one to type in the club name and then print it.

I envision this bookplate as a recognition for anyone who has donated a book,  This could be a club or club member or even an out of family donor. The recognition area should be available for typing as suggested by Gail Heidbrink and perhaps adjust to fit a standard Avery label.  Congratulations on a great idea..

Check this out: http://community.kiwanisone.org/media/p/8802.aspx Here is the format for a standard avery label with the specs so you can identify which label to buy. This should solve both of your issues!
